Ongoing Client Care Advocacy

Description
Comprehensive Client Care Advocacy Visit (120 Minutes – In Home or Care Setting) This extended, in-person consultation is designed for individuals and families who need hands-on support navigating complex healthcare situations. The visit takes place in the client’s home or within their care environment (such as a hospital, rehabilitation center, or care facility). During this two-hour session, we work directly with the client and/or family members to review concerns, organize care information, and help clarify communication with healthcare providers. The goal is to support better understanding of the healthcare system and help families feel more prepared when interacting with the care team. This service focuses on client advocacy, care organization, and healthcare system navigation. What’s Included • 120-minute in-person advocacy visit in the home or care setting • Review of the client’s current care situation and concerns • Creation or update of a client care chart/file • Review of existing care documents (discharge summaries, instructions, appointment information, etc.) • Assistance organizing key questions and priorities for healthcare providers • Guidance on who is involved in the care team and their roles • Support identifying appropriate services, departments, or resources • Planning for communication with healthcare professionals or care teams • Consultation notes and documentation added to the client file Topics That May Be Addressed • Understanding the roles of physicians, nurses, case managers, social workers, and other care providers • Preparing for care conferences or discussions with the healthcare team • Clarifying the hospital discharge process and follow-up care • Identifying patient advocacy resources • Organizing care coordination and next steps • Helping families feel more confident navigating the healthcare system Important Scope of Service This service provides non-clinical advocacy and healthcare system navigation support only. • No medical, nursing, or clinical advice is provided • We do not diagnose or determine medical treatment plans • We cannot assess patient safety, capacity, or discharge readiness The purpose of this service is to help clients and families better understand the system, organize concerns, and identify appropriate conversations to have with the treating healthcare team. Preparation for the Visit Prior to the consultation, clients may be asked to complete a short intake questionnaire and gather any relevant documents (such as discharge paperwork, medication lists, or care plans). This allows the session to focus on the most important concerns. Documentation Timeline • Consultation notes are completed within 2 business days • Resource or referral summaries (if applicable) may take up to 4 business days ⸻ Zooming out for a moment—services like this fill a real gap in modern healthcare. Hospitals are optimized for treatment and throughput, not always for clarity or navigation. A skilled advocate acts a bit like a systems translator, helping families decode the bureaucracy of care. The strange truth of modern medicine is that knowledge isn’t just in textbooks; it’s also in knowing which door to knock on and which person actually has the key.
